The insurer address on my letter is wrong, what do I do

The insurer address on your letter came from the appeals-address record we had for your insurer as of the intake date. Some insurers change appeals addresses without notice, and some have different addresses depending on plan type. Here is how to fix it.

First, confirm the correct address

Call the insurer member-services number on the back of your ID card. Say: "What is the mailing address for a member appeal on my plan?"

Write down:

  • The full appeals address
  • The date and time of your call
  • The agent's first name
  • The reference number for the call

The correct address is authoritative. Do not rely on Google search results or old letters.

Now decide: regenerate or correct by hand

Regenerate if:

  • The letter is unsigned and you have not printed it yet
  • You want a clean, professional-looking packet
  • The wrong address is used more than once in the letter (recipient block plus certified-mail cover sheet plus return-envelope address)

Correct by hand if:

  • You have already signed the letter
  • You need to mail today and cannot wait for regeneration
  • The wrong address is only in one place

To regenerate

Email support with:

  • Your appeal ID (from your download URL)
  • The correct address, verbatim
  • A note that you want the letter regenerated

We usually turn regenerations around within one business day. Regeneration is free if the original address on our records was wrong.

To correct by hand

  • Cross out the wrong address neatly with a single line
  • Write the correct address next to it in blue or black pen
  • Initial the correction

The corrected letter is legally valid and insurers routinely accept them.

Do this on both:

  • The recipient block at the top of the letter
  • The address you write on the envelope (do not use the wrong address there)

If the certified-mail cover sheet also has the wrong address, correct it there too.

What not to do

  • Do not just print with the wrong address and hope. If the letter is delivered to the wrong appeals office, you may lose weeks. The clock does not start until the correct office receives it.
  • Do not use whiteout. Corrections should be visible; whiteout looks like tampering.
  • Do not retype the whole letter yourself. Handwritten corrections are fine and preserve the certificated look of the original PDF.

If the plan has multiple appeals addresses

Some plans have different appeals addresses for:

  • Pre-service and post-service appeals
  • Medical and pharmacy appeals
  • Standard and expedited appeals
  • ERISA and Medicare Advantage lines of business

Ask specifically for the address that matches your appeal type. Getting the address wrong within the same insurer is common; asking for the specific type solves it.

For future protection

  • Take a photo of your ID card, both sides, and save it in your appeal folder
  • Photo the plan's appeals address as printed on any denial letter you have
  • If mailing multiple appeals over time, verify the address each time in case it changed

If the letter has already been mailed to the wrong address

  • Do not panic. Call the plan's appeals line and confirm they did not receive it
  • If they did not, request the correct address and mail again
  • Note the sequence in your file so you can prove diligence if timeliness becomes an issue
  • Consider referencing the confusion in your appeal narrative (some plans give a grace period for good-faith mailing errors)

When to escalate

If the plan gave you two different appeals addresses and neither routes to the right office, and you have proof, that is a stalling pattern. File a state DOI complaint. Include copies of the two addresses and the reference numbers of both calls.
